Course description

This module can be undertaken as an individual module or altogether as a package on the EI’s learning management system, with recorded presentations and exercises taught by a leading expert in process safety and a former deputy director of the UK Health & Safety Executive.

Learning Objectives:

  • Understand how PTW systems fail and lead to major accidents
  • Understand the different roles and functions necessary within a PTW system

Training Course Content

Key Points to cover:

Purpose: To ensure high-risk maintenance activities are controlled to safeguard people and the integrity of plant and equipment.

Learning Points:

  • PTW system is a permissioning system
    • Permission is conditional upon appropriate safeguards being in place before work starts
    • People should not give themselves permission
    • Risk assessment methods to be used with each type of work clearly set out
    • Thinking beyond the ‘tick boxes’ on PTW forms
    • Scope & types of permissions and permits clearly set out
  • Roles & responsibilities clearly set out
  • Instruction & training of permit issuers and users
  • Permission is recorded in a documented permit(s)
    • Method of isolations set out (pre-determined for key plant / processes)
    • Walk through of precautions with workers before work starts – risk communication
    • Validation of isolations & precautions before work starts
    • Lock of Tag off system in use
    • Instruction to stop work if anything unexpected occurs or dangerous situation exists
  • Permit suspension if work can’t be completed in period set out
  • Permit handover between shifts
  • Live permits available in operations control room and maintenance department
  • Suitable re-instatement and hand back arrangements
  • Compliance with permit conditions routinely checked whilst work is underway
