Introduction
Recent years have seen a series of high-profile coroner’s inquests making the headlines nationally. On a wider level, inquests have continued to throw up challenging issues for interested persons including the bereaved, other individuals, private and public organisations involved. In particular, organisations risk reputational damage and the prospect of negative reports by coroners to prevent further deaths (’PFD’) in accordance with regulation 28 of the Coroners (Investigation) Regulations 2013 with further changes now on the horizon.
